    Background:

    FamilyWell’s mission is to improve access to high-quality, equitable, and affordable perinatal mental health care for pregnant and postpartum individuals in the U.S. FamilyWell is a behavioral health company that embeds tech-enabled perinatal mental health services including coaching, therapy, and psychiatry directly in medical clinics by leveraging the evidence-based collaborative care model (CoCM). CoCM has been validated in over 90 randomized controlled trials and adopted by Medicare, Medicaid, and many commercial plans across the country. The CoCM model allows medical providers to bill insurance companies for mental health services embedded in their clinics. FamilyWell partners with OB, Family Medicine, Internal Medicine practices to provide mental health prevention, detection, and treatment for pregnant and postpartum individuals struggling with perinatal mood & anxiety disorders (PMADs).

    Role:

    FamilyWell’s CoCM care team includes behavioral care managers (BCMs), therapists, coaches, and psychiatric consultants. The role’s primary focus is ongoing support sessions for patients who are on the therapy pathway. Additionally, patient intakes may be part of the role as needed.The licensed therapist reports to the Director of Clinical Operations with support from the Lead Therapist. All duties are virtual and there is no in-person requirement.

    Requirements:

    • Licensed independent mental health provider who holds an active license in the state of Massachusetts.

    • Qualified applicants must have formal perinatal mental health training or at least 2 years of clinical experience working with pregnant and/or postpartum patients.

    • Licensures that qualify:

      • Licensed Independent Clinical Social Worker (LICSW)

      • Licensed Mental Health Counselor (LMHC)

      • Licensed Marriage Family Therapist (LMFT)

    • Recommended (but not required): Perinatal Mental Health Certification (PMH-C) from Postpartum Support International

    Duties and Responsibilities:

    *All responsibilities are performed virtually by telehealth.

    • Provide individual tele-therapy (virtually only) for patients requiring or requesting regular one-on-one sessions using FamilyWell’s virtual platform and provide Support Plans post-session to patients.

    • Conduct one-on-one psychological intakes for referred FamilyWell program patients.

    • Document patient progress and treatment recommendations to be shared with medical providers, psychiatric consultants, and other treating providers.

    • Facilitate treatment plan changes for patients who are not improving as expected and who may need more intensive or more specialized mental health care.

    • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.

    Background:

    FamilyWell is the first digital health company in the U.S. to provide tech-enabled perinatal behavioral health services through an evidence-based collaborative care model (CoCM). The collaborative care model has been validated in over 90 randomized controlled trials and adopted by Medicaid and many commercial plans across the country, including those in Massachusetts The CoCM model allows medical providers to bill insurance companies for perinatal behavioral health care services. FamilyWell partners with medical providers (OB, Family Medicine, Internal Medicine) to provide mental health prevention, detection, and treatment for pregnant and postpartum patients, with a strong focus on the Medicaid population.

    Role:

    The psychiatric consultant supports the collaborative care team, which includes the OB or Family Medicine providers, patients, and behavioral care managers (licensed therapists, social workers, coaches)—through weekly consultations on an assigned caseload of patients. The psychiatric consultant advises the OB provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions.

    This is a contractor position starting at 5-10 hours per week (with potential to increase to full-time). All duties are virtual and there is no in-person requirement.

    Requirements:

    • Licensed psychiatrist or psychiatric nurse practitioner in the state of Massachusetts

    • Ability to quickly synthesize medical and psychiatric data and formulate effective and evidence-based clinical recommendations

    Duties and Responsibilities:

    • All responsibilities are performed virtually by telehealth, email, telephone, or text. There are no in-person requirements and the consultant may work exclusively from home.

    • Provide regularly scheduled (usually weekly) caseload consultations identified by the behavioral care manager. Case review consultations focus primarily on patients who are new to the caseload or who are not improving as expected under their current treatment plan. Case reviews may be conducted by telephone, video, or by email.

    • Work with behavioral care managers to oversee the status and population health of their assigned patient caseload. The psychiatric consultant will use FamilyWell’s registry to track all assigned patients and prioritize patients for case review and treatment adjustment.

    • Recommend any treatment adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions, within 24 hours of consulting with a behavioral care manager or other team member.

    • Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the medical setting until patients can be engaged in specialized care as appropriate.

    • Clearly communicate the limitations of the case review consultation and treatment recommendations.

    • Be available to provide occasional ad hoc telephone, email, or telehealth consultations to primary care providers for very complex patients or challenging patient situations. Respond to telephone calls from primary care providers and behavioral care managers within one business day. Respond to urgent telephone calls within one hour, if available. Coordinate with other psychiatric consultants for vacation coverage.

    • Direct evaluation of patients with significant diagnostic or therapeutic challenges who are identified in discussion with the patient’s behavioral care manager and/or primary care provider. Such consultation may be provided in person or via telemedicine (video or telephone).

    • Maintain professional cell phone and email accounts for contact during usual business hours. Check professional accounts daily and respond to email questions/consultations within two business days, sooner if urgent.

    Time Commitment: Contractor, 5-10 hours per week with potential to increase to full-time. All duties are virtual and there is no in-person requirement.

    Role:

    The Behavioral Care Manager (BCM) is a core member of the collaborative care team, which also includes coaches, licensed therapists, psychiatric consultants, and OB/Gyn clinical staff. The BCM is responsible for processing referrals, patient engagement and recruitment, educating and enrolling patients in the program, reviewing eligibility, administering and following up on monthly mental health screenings, insurance verification, scheduling patient sessions, updating patient records, and communicating directly with the team. The BCM will be based in-person at the partnering OB/Gyn clinic(s), and interact directly with clinic staff, providers, and patients. The BCM will report directly to the Director of Perinatal Behavioral Health.

    *This is a hybrid role that requires duties to be performed in-person, within an OB clinic(s) in Hillsborough County, New Hampshire, and virtually.

    Requirements to be considered for an interview:

    • LICSW

    • 2 years of previous experience providing therapy for perinatal mental health patients

    • Lives in Hillsborough County, New Hampshire or within 30 mile radius

    *Ideal candidates will have completed (or are in the process of completing) their Perinatal Mental Health Certification (PMH-C) through Postpartum Support International and will be bilingual in Spanish (and/or Brazilian Portuguese) and English.

    Duties and responsibilities:

    • Directly engage with pregnant and postpartum patients in OB clinics to actively educate, recruit, and enroll eligible patients in FamilyWell’s perinatal collaborative care program.

    • Review patient eligibility, verify insurance coverage, and complete all enrollment requirements.

    • Communicate directly with patients (either in person, by text, or by phone) for program recruitment outreaches, reminders, and follow-up care to maintain a goal of 80% patient engagement in the program.

    • Update patient records in EHRs and communicate care plans to the clinic providers.

    • Maintain a collaborative care registry provided by FamilyWell. Track patient follow up and clinical outcomes in the registry.

    • Prepare and submit monthly patient progress reports in EHR.

    • Send mental health screenings, reminders, and follow-up on results.

    • Provide patient education about common perinatal mental health disorders and the available treatment options.

    • Facilitate outside referrals for community-based social services (e.g. housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment) as clinically indicated.

    • Compose, prepare, and communicate timely patient and provider responses to questions.

    Time Commitment:

    • Full-time, 30-40 hours/week
